GENERAL INFORMATION

     Thank you for visiting the web site of Dr. Mark K. Holcomb, Clear Lake Oral and Maxillofacial Surgery. This page provides important information about our practice. Here we have tried to anticipate your questions in order to address your concerns.

     Our professional staff is well trained and highly qualified to assist you with the highest quality of care. It is our goal to make your visit with us a comfortable experience.

     Upon your arrival you will be asked to complete some patient forms as part of your health record. This information is essential for us to treat you as a patient. If you have a complex medical history, it would be helpful if you have a list of medications in which you are currently taking. If you have a medical condition such as: artificial heart valve(s), artificial joint(s), heart murmur, rheumatic fever or mitral valve prolapse, please share that information with us so that the required pre-medication can be prescribed. If you are taking a blood thinner, it may be necessary to consult with your internist prior to your oral surgery. Your medical history will be discussed at the time of your consultation. A copy of the health questionnaire is available for your completion prior to your scheduled appointment.

     It would assist us if you have a referral slip and a panoramic x-ray from your dentist at the time of your consultation. You do not have to be referred to our office by your dentist. Many of our patients are self-referred. However, with some insurance carriers, it is required that you have a referral from an in-network dentist for coverage of treatment. If a panoramic x-ray is not available, we will take one when you are seen.

     If your child is a minor, it is essential for at least one parent or legal guardian to be present at all appointments.

     Because of the surgical and anesthetic treatment, we require that our patients be seen before the actual surgery for a consultation to explain and discuss the options for surgery and anesthesia. Patients receiving IV anesthesia will be required to fast prior to surgery. However, there may be exceptions depending on your medical history. Pre-operative and post-operative instructions are available for your convenience.
