If you are having problems viewing or do not have Adobe Acrobat Reader:
It would assist us if you have a referral slip and a panoramic x-ray from your dentist at the time of your consultation. You do not have to be referred to our office by your dentist. Many of our patients are self-referred. However, with some insurance carriers it is required that you have a referral from an in-network dentist for coverage of treatment. If a panoramic x-ray is not available, we will take one when you are seen.
If you have a complex medical history, it would be helpful if you have a list of medications in which you are currently taking. If you have a medical condition such as: artificial heart valve(s), artificial joint(s), heart murmur, rheumatic fever or mitral valve prolapse, please share that information with us so that the required pre-medication can be prescribed.
If your child is a minor, it is essential for at least one parent or legal guardian to be present at all appointments.
