FINANCIAL INFORMATION

     We are committed to providing you with the best possible care, and we are pleased to discuss our professional fees with you at any time. Your clear understanding of our financial policy is important to our professional relationship. Please ask a staff member if you have any questions about our fees, financial policy, or your responsibility.
  • Full payment is due at the time services are rendered, unless prior arrangements have been previously made.
  • We accept cash, check and all major credit cards with proper identification.
  • Other outside sources are available for patients who wish to finance their treatment.
     We will do all we can to assure that you receive the maximum benefits to which you are entitled under your insurance carrier or dental plan. Dr. Holcomb's diagnosis and recommended treatment is based on his clinical findings of your examination and x-rays, not upon what your insurance plan will or will not cover.

     We accept most major insurance plans. For your convenience, we will file your insurance claim with your primary insurance carrier. We do not file secondary insurance. However, we will provide you with the necessary information for submission of reimbursement. You will be expected to pay all co-pays and estimated portions at the time services are rendered.

     The amounts listed as to what your insurance carrier may pay are "good faith estimates" based on the limited information your insurance carrier has provided us. The actual amount your insurance carrier pays depends on the specific criteria of your insurance plan. Dental and medical insurance plans may vary widely and we feel the information we have included below may be helpful.
  • Dental/Medical insurance will not cover all of your treatment.
  • Many carriers will tell their insured (you, the patient) that they will cover "up to 50%, 80% or up to 100%". In spite of what you are told, we have found that most plans cover about 50%-75% of an average fee.
  • Some plans pay more...some less. The amount your plan pays is determined by how much you or your employer paid for the plan. The less paid for insurance, the less you will receive in benefits.
  • Some insurance companies tell their customers that the fees charged are above the "usual and customary fee" rather than saying that their benefits are low. Remember, you get back only what you or your employer puts in less the profit of the insurance carrier.
  • Not all services are covered by your carrier due to plan limitations or exclusions.
     Since information given over the phone does not guarantee coverage or payment, we strongly recommend a written pre-determination prior to treatment in order to know the exact benefits your insurance carrier will pay.

     If your insurance carrier pays more than the amount we estimated, we will send you a refund. If your insurance carrier pays less than the amount we estimated, you will be responsible for the balance.

     Dr. Holcomb has opt-out of the Medicare program. We do not file or accept Medicare. Therefore, all Medicare beneficiaries will be required to sign a private contract prior to any treatment. Full payment will be due at the time services are rendered.
